Medical Consent

Please read this Medical Consent carefully before using the MDMD platform. By proceeding with a telehealth consultation through MDMD, you acknowledge that you have read, understood, and agree to the terms described below.
1. Nature of Telehealth Services
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a patient and a healthcare provider who are not in the same physical location. Through the MDMD platform, you may receive clinical evaluation, diagnosis, treatment recommendations, and prescriptions from an independent, licensed healthcare provider (“Provider”) via asynchronous (store-and-forward) communication or, where required by state law, synchronous (live audio/video) consultation.
MDMD is a technology platform and does not itself provide medical services. Your Provider is an independent licensed clinician and is solely responsible for all clinical decisions.
2. Limitations of Telehealth
You understand and acknowledge that telehealth has limitations compared to in-person medical care, including but not limited to:
- The Provider will not be able to conduct a physical examination.
- The Provider will rely on the information you provide, which must be accurate and complete. Incomplete or inaccurate information may affect the quality of care.
- Technical failures (internet disruption, equipment malfunction) may interrupt or prevent the consultation.
- Telehealth may not be appropriate for all medical conditions. Your Provider may determine that an in-person visit is necessary.
3. Compounded Medication Informed Consent
If your Provider determines that a compounded GLP-1 receptor agonist medication is appropriate for you, you acknowledge and understand that:
- Compounded medications are not FDA-approved. They have not been evaluated by the FDA for safety, efficacy, or quality.
- Compounded medications are prepared by licensed U.S. compounding pharmacies in accordance with applicable state and federal regulations.
- Compounded medications may differ from commercially manufactured branded medications in terms of potency variability and quality control. You acknowledge and accept these differences.
- The decision to prescribe compounded medication is made at the sole discretion of the licensed Provider assigned to your case.
- Potential side effects of GLP-1 receptor agonist medications may include (but are not limited to): nausea, vomiting, diarrhea, constipation, abdominal pain, headache, fatigue, dizziness, injection site reactions, and in rare cases, more serious adverse events including pancreatitis, gallbladder problems, kidney problems, and allergic reactions.
4. Right to Refuse Treatment
You have the right to refuse or discontinue treatment at any time, for any reason. If you choose to refuse or discontinue treatment, your Provider may discuss the potential consequences with you, but the final decision is yours. Refusal of treatment will not affect your ability to seek care elsewhere.
5. Emergency Disclaimer
MDMD is not an emergency medical service.
If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use the MDMD platform or wait for a response from an MDMD Provider in an emergency situation.
6. Accuracy of Information
By submitting your intake form and proceeding with a telehealth consultation through the MDMD platform, you confirm that all information you have provided is accurate and complete to the best of your knowledge. Providing inaccurate or incomplete information may affect the quality and safety of your care.
7. Consent Acknowledgment
By submitting your intake form, you confirm that:
- You have read and understand this Medical Consent.
- You understand the nature, limitations, and risks of telehealth services.
- You understand that compounded medications are not FDA-approved and may differ from branded medications in potency and quality control.
- You voluntarily consent to participate in telehealth services through the MDMD platform.
- You understand your right to refuse or withdraw consent at any time.
- You confirm that all information provided is accurate and complete.
